Our study has demonstrated through analysis of the National Surgical Quality Improvement Program that inferior mesenteric artery (IMA) replantation does not sufficiently decrease the rates of ischemic colitis after an open abdominal aortic aneurysm (AAA) repair that is comparable to the cohort of patients who undergo IMA ligation.1 Lazaris and colleagues have expressed in their letter to the Editor that comparison of these two cohorts (open AAA repair with IMA replantation vs open AAA repair with IMA ligation) is not valid because of inherent underlying differences.

Lee et  al1 report that inferior mesenteric artery (IMA) replantation does not decrease the risk of ischemic colitis after open infrarenal abdominal aortic aneurysm (AAA) repair. On the contrary, when there are intraoperative findings supporting IMA replantation, despite this additional procedure, the ris k of colonic ischemia is higher than after AAA repair without IMA replantation.

Multiple renal arteries are a common renal vascular variant. A study with 800 kidneys from cadaver donors demonstrated 1% prevalence of quadruple renal arteries.1 During thoracoabdominal aortic aneurysm (TAAA) repair, this anomaly implies longer time of renal ischemia and well-defined reperfusion strategy.2 Acute kidney injury during TAAA repair is a common complication with 6% to 7% incidence.3,4 It can be prevented by histidine-tryptophan-ketoglutarate solution (Custodiol; Dr Franz-K öhler Chemie GmbH, Bensheim, Germany) renal perfusion.

Conclusions: We provided evidence that ARID2 knockout could contribute to disruption of NER process through inhibiting the recruitment of XPG, resulting in susceptibility to carcinogens and potential hypermutation. These findings have far-reaching implications for therapeutic targets in cancers harboring ARID2 mutations.
