In patients with persistent subtypes of atrial fibrillation (AF), the left atrial appendage (LAA) can be a critical source of nonpulmonary vein triggering. The addition of empirical isolation of the appendage in patients with persistent AF has been shown to improve arrhythmia-free outcomes compared to standard-of-care ablation approaches. The LAA can be challenging to isolate and often requires higher power settings and extensive ablation to create durable isolation. Ibrahim et  al (DOI: https://doi.org/10.1016/j.hrcr.2018.06.007) shared 2 cases from LEIO-AF (LAA Electrical Isolation and Occlusion to Treat Persistent AF) study in which failure to isolate the appendage was due to coronary sinus connections.
