The Changing Epidemiology of Ventricular Arrhythmias and Sudden Cardiac Death: Is it a Reality?

A few reports have suggested that the risk of sudden cardiac death (SCD) may be decreasing in patients with systolic heart failure.1-3 While this may be true and can be explained by improved pharmacologic and device-based therapies, it is important to keep in mind the context in which this decreasing risk of SCD was observed. The vast majority of data on the declining risk of SCD were generated in randomized clinical trials which notoriously enroll “healthier” patients receiving excellent medical therapy that is often hard to achieve in clinical practice.
