Biomarkers in Chronic Rhinosinusitis with Nasal Polyps

Chronic rhinosinusitis is a complex disease that exists along the inflammatory spectrum between types 1 and 2 inflammation. The classic phenotypic differentiation of chronic rhinosinusitis based on the presence or absence of inflammatory polyps remains one of the best differentiators of response to therapy. Development of biologics for the treatment of atopic disease and asthma and topical therapies for sinusitis have placed renewed emphasis on understanding the pathophysiology of polyp disease. Identification of key markers of polyposis will allow for better stratification of inflammatory polyp disease endotypes to objectively identify medical therapies and track response to treatment.

Conclusions: Allergic rhinitis is common comorbidity of asthma that interrelated to asthma control. Identifiable allergens could be detected AR in asthma by SPT. Therefore, reduction of allergens exposure could become preventive approach for asthma patients with AR.

Conclusions: The severe asthmatic patients examined had a high prevalence of Aspergillus sensitization and of Aspergillus-associated diseases as ABPA (26%) and CPA (26%). It has to be demonstrated the benefit of an early recognition and treatment of these diseases on the course of asthma.

Aspirin-exacerbated respiratory disease (AERD) is a syndrome characterized by asthma that is often severe with progressive airway remodeling, the presence of eosinophilic chronic sinusitis with nasal polyp (CRSwNP) formation, and intolerance to aspirin and other non-selective cyclooxygenase 1 (COX-1) inhibitors.1 The prevalence of AERD in the general population is 1% which increases to 7% amongst those with asthma. In the most severe cases, ingestion of aspirin can trigger systemic reactions and even death.
