More on the pacemaker ECG in severe hyperkalemia

We thank Tomcs ányi and Bózsik [1] for their interest in our work [2] and allowing us to further elaborate on electrocardiographic (ECG) signs of severe hyperkalemia. We agree that in patients with pacemakers, conventional ECG markers of hyperkalemia are frequently lacking and therefore, any sign that can lead t o this important clinical diagnosis is welcome. As Tomcsányi and Bózsik have pointed out, well established manifestations include marked widening of the paced QRS complexes [3] as well as any type of failure of pacemaker sensing and capture [4].
