Nocturnal motor behaviors with unexpected EEG and brain MRI findings

Arousal Disorders (DOA) are usually childhood parasomnias. When DOA persist during adulthood they can be difficult to distinguish from other nocturnal behaviors, particularly sleep-related hypermotor epilepsy (SHE). We observed a patient with nocturnal behaviors clinically suggestive of DOA in which EEG and brain MRI features unexpectedly posed a challenge in the differential diagnosis with SHE. A 28-year-old woman complained of abrupt nocturnal awakenings followed by sleepwalking and sometimes running out of the room.
