Conclusions A minimally invasive surgery through mini right anterior thoracotomy is a good alternative technique for treating cardiac myxoma. Despite the small size of the experience, there is a clear diminution in preoperative blood loss and an interesting trend toward a shorter intensive care unit and hospital stay.

CONCLUSIONS: CLE is a rare malformation with variable presentation. Infants presenting with respiratory distress or recurrent chest infection should be evaluated for the possibility of CLE. Lobectomy is the treatment of choice, and rarely, the patients may be managed conservatively. Thoracoscopic lobectomy is a safe procedure with the possibility of air leak and conversion to open lobectomy.

The last decade has seen a transition in the surgical approach to lobectomy, from thoracotomy to minimally invasive surgery (MIS), for treating patients with early-stage non-small cell lung cancer (NSCLC) [1]. In this issue of the Journal, Kneuertz and associates1 have reported a 2-institution study comparing lymph node upstaging by means of different surgical platforms: thoracotomy, video-assisted thoracoscopic surgery (VATS), and robotic surgery. The study is well done, and the data and statistics are also well done; however, the question asked cannot be answered by this study, nor will it be by any study soon. The reason is simple —the degree of lymph node removal during thoracic cancer surgery comes down to the thoracic surgeon's mind-set more so then the platform he or she chooses for surgery.
