Preferences for Toxicity Monitoring of Patients on Abiraterone Acetate Plus Prednisone

Madam – Abiraterone acetate in combination with prednisone or prednisolone (AA+P) is one of several novel oral hormone therapies that is effective in treating prostate cancer and is well tolerated [1]. Common adverse reactions include peripheral oedema, hypokalaemia and hypertension, which are easily tr eated but require careful monitoring [2,3]. At University College London Hospital (UCLH) this involves additional hospital visits, twice every 2 weeks for the first 3 months and thereafter twice every 4 weeks, in addition to routine clinic appointments for prostate-specific antigen checks and consul tations.

CONCLUSIONS: Abiraterone acetate is a new hormonal treatment for metastatic castration resistant prostate cancer both before and after chemotherapy. The results of the available studies demonstrate a significant improvement in terms of efficacy, with a tolerability profile generally acceptable, predictable and manageable, and an improvement in patient's perceived quality of life. Prostate cancer, a neoplasm that commonly causes bony metastases, rarely involves the jaw. An 80-year-old man presented with complaints of pain and paresthesia on the right side of the mandible for 3 months. Anamnesis revealed systemic hypertension and prostatic hyperplasia. Clinical examination revealed a symptomatic nodule involving the right mandibular posterior region. Panoramic radiography and computed tomography analysis showed periosteal reaction at the inferior border of the right mandible.

Abiraterone acetate (AA) is a potent selective inhibitor of cytochrome P450 (CYP) 17, a key enzyme involved in testosterone synthesis. Due to this inhibition, the production of androgens by endocrine tissues decreases. Therefore, this oral hormone therapy is used in castration-resistant prostate cancer combined with prednisone/prednisolone, with a significant increase in overall survival. Common side effects of AA include hypertension, hypokalaemia, peripheral oedema and urinary tract infections.
