Rare Collateral Pathway in a Patient with Aortoiliac Occlusive Disease

A 66-year-old man with a history of smoking, hyperlipidemia, and significant carotid artery disease presented to the interventional radiology clinic with labile hypertension and concurrent renal artery stenosis. On further workup, he was noted to have aortoiliac occlusive disease since the 1990s with associated claudication that improved with exercise. During workup of peripheral vascular disease, a computed tomographic angiogram was obtained, which revealed a large hepatic falciform artery collateralizing with the right inferior epigastric artery via a rare collateral vessel (Fig 1).
