Clopidogrel Hypersensitivity: Overview of the Problem. Curr Vasc Pharmacol. 2018 Aug 20;: Authors: Savage MP, Fischman DL Abstract Clopidogrel is one of the most commonly prescribed drugs worldwide. It plays an essential therapeutic role in a variety of vascular conditions including acute coronary syndromes, ischemic heart disease treated with coronary stents, stroke, and peripheral vascular disease. Hypersensitivity reactions to clopidogrel are not uncommon and lead to its discontinuation in 1-2% of patients. Clopidogrel hypersensitivity is especially problematic in patients with newly implanted drug-eluting coronary stents. Premature discontinuation of dual antiplatelet therapy after recent coronary intervention is the most important risk factor for stent thrombosis. Clopidogrel hypersensitivity most often occurs during the first week after initiation, when drug discontinuation carries the highest risk for stent thrombosis. Since stent thrombosis typically results in a myocardial infarction or death, the management of clopidogrel hypersensitivity reactions is a clinical issue of great importance. This study aimed to evaluate the prognostic role of NLR, PLR, and LMR in PE. A total of 103 PE cases from a cardiology department were included in the study. Control group consisted of 102 patients selected from outpatient clinics other than cardiology, cardiovascular surgery, and chest diseases. We present a death from acute pyelonephritis in a setting of hypertensive heart disease, in which the unenhanced PMCT performed before postmortem examination was able to show florid bilateral perinephric fat stranding. This PMCT finding subsequently guided the pathologist to pay special attention to the kidney and take appropriate microbiology specimens to confirm the diagnosis. Perinephric fat stranding on PMCT may be a useful sign to alert the pathologist to consider acute pyelonephritis during the postmortem examination.
