Hydroxychloroquine-induced mimic of renal Fabry  disease

A 27-year-old Asian woman was evaluated for a 15-year history of systemic lupus erythematosus treated with hydroxychloroquine, azathioprine, and intermittent prednisone. For the preceding 2 years, the urine protein/creatinine ratio fluctuated between 0.4 and 3.8 g/g with mild pyuria (5 –31 white blood cells/high power field). The estimated glomerular filtration rate was 119 ml/min per 1.73 m2. Kidney biopsy demonstrated mesangial proliferation and membranous glomerulopathy consistent with International Society of Nephrology/Renal Pathology Society Classification II + V.
