There's a time for everything

Cardiovascular diseases show diurnal variation, with a higher incidence of adverse events in the early morning than in the evening. Traditionally, this diurnal variation has been ascribed to extrinsic (extracardiac) factors, including posture, physical exertion, food consumption, drug intake, temperature and light exposure, influencing the neuroendocrine, metabolic and prothrombotic status of a patient [1]. Among environmental conditions, also a less efficient medical response during the night has been hypothesized in the reported worst outcomes occurring early in the morning.
