Neighborhood Navigator to address Social Determinants of Health

The EveryONE project is an initiative from the American Academy of Family Physicians focused on helping primary care physicians learn about and address the social determinants of health (SDOH). The Neighborhood Navigator is an output of this project. The navigator allows a user to search for services available in their zip code that will help them with issues such as hunger, paying for medical care and job training. It is important that SDOH are addressed alongside an individual’s physical health needs and this navigator offers a place to start in addressing them.
