Brugada syndrome: Keep an eye on the electrocardiogram

Since its identification in 1992, Brugada syndrome has been the subject of intense research to identify the factors involved in the occurrence of sudden cardiac death to better target patients requiring medical care.1 This pathology is atypical in that it does not induce cardiomyopathy but leads to sudden death in the vast majority of patients who were previously completely asymptomatic since 4 of 5 patients who die of Brugada syndrome had not experienced any prior symptoms.2

We report a case of an 8-year-old girl who presented with syncopal attacks and a history of viral illness a month ago. On examination, she was conscious, oriented but had a heart rate of 42/min which was unresponsive to atropine. She was started on dobutamine and isoproterenol. Described herein are certain clinical and morphologic findings in 9 patients who at necropsy had hearts weighing>1000g, a weight approximately 3 times normal. With the exception of 2 patients with hypertrophic cardiomyopathy, the common finding in the remaining 7 patients was obesity. None had valvular heart disease, the previously described major cause of massive cardiomegaly. Thus, obesity needs to be added to the causes of massive cardiomegaly, a cause not previously recognized. Electrocardiograms in 4 patients disclosed high total 12-lead QRS voltage on the electrocardiogram in only one despite the massive cardiomegaly.

CONCLUSIONS: Screening for IDCM should include a three-generation family history and clinical screening of all first-degree family members. As IDCM has an age-related penetrance, at-risk family members should receive follow-up for screening to assess symptoms and signs of IDCM.
