Widespread elastosis perferans serpiginosa secondary to tiopronin

A 70-year-old lady presented to the dermatology clinic with a left shoulder lesion which did subsequently spread and involved both her buttock area. The lesions were asymptomatic most of time. Occasionally they became pruritic. She was otherwise well with a history of cystinuria, hypertension, asthma, pulmonary hypertension and brochiectasis for which she was on multiple medications including Thiola (tiopronin) tablets. On examination she had multiple annular lesions with a rim of erythema spread throughout her trunk.
