A 62-year old non-smoker, Caucasian male was evaluated for lung transplantation based on severe upper-lobe predominant emphysema with superimposed fibrosis in the lower lung zones (Figure 1). The patient had no significant exposure history, and prior work-up for alpha-1-antitrypsin deficiency, autoimmune disease, and hypersensitivity pneumonitis was unremarkable. The diagnosis of Niemann Pick disease, type B was eventually established based on a compatible family history with an almost identical disease presentation in a sibling.

Infections of the driveline exit site are very common and are one of the most common reasons for readmissions of patients with left ventricular assist devices (LVAD). The risk for LVAD infection increases with the duration of LVAD support and after 48 months it is among the top three reasons for death [1-3]. We describe the use of a decontamination protocol that allowed for successful lung transplantation in a cystic fibrosis patient with necrotizing pneumonia from highly antibiotic-resistant pathogens (Burkholderia and psuedomonas species). This strategy may allow for successful lung transplantation in cystic fibrosis patients with multi-drug resistant infections previously considered non-transplantable.
