Optimization of Prostatic Cryosurgery with Multi-Cryoprobe based on Refrigerant Flow

In this research, we applied a transient 3D two-phase refrigerant flow model inside the LN2 boiling chamber as well as a bioheat transfer model inside the tissues to evaluate the optimized ablation outcome during prostatic cryosurgery. For the evaluation of the scenarios, a defect function was used that considers non-ablated target tissue (prostate/cancer tissue) as well as ablated healthy tissue, in which the ablated tissue was evaluated using a temperature threshold. Three different configurations using three LN2 cryoprobes were analyzed during the modeling study, and the best configuration with the three LN2 cryoprobes positioned isoscelesly was found. For this configuration, temperature distributions and temperature profiles at specific points within the tissue were investigated numerically. Owing to its low computational cost, the 3D coupled model has an advantage in accurate modeling cryosurgery for curing numerous diseases.

We report a case of collision metastasis of prostate adenocarcinoma and urothelial carcinoma of the bladder in a 73-year-old man who underwent cystoprostatectomy with bilateral pelvic lymph node dissection for high-grade muscle-invasive urothelial carcinoma. Final pathology revealed a pT3aN2 high-grade urothelial carcinoma and pT3N1 Gleason 4 + 4 = 8 adenocarcinoma of the prostate with 12/40 pelvic lymph nodes positive for urothelial carcinoma. We report a case with prostate cancer and gastric neuroendocrine carcinoma. A 72-year old male presented with a gastric lesion 5 months after radical prostatectomy. The lesion was immunohistochemically positive for PSA, alpha-methylacyl-CoA racemase, synaptophysin, and chromogranin A, but negative for androgen receptor (AR). We report a patient that had a prior radical prostatectomy and negative PSA levels for two years and subsequently developed bladder cancer requiring radical cystectomy with mixed lymph nodes on final pathology. The nodes were found to be positive for both metastatic urothelial cell carcinoma and metastatic prostatic adenocarcinoma based on immunohistochemical staining. Treatment for metastatic bladder cancer was pursued after radical cystectomy recovery.

We examined the Arg1-positive cells in tumor microenvironment of gastric carcinomas (GCs), colorectal carcinomas (CRCs) and prostate carcinomas (PCs), and analyzed their clinicopathological significance. Immunohistochemical staining for Arg1 was done in 60 GCs, 38 gastric adenomas, 40 CRCs, 10 colonic adenomas, 36 PCs, and 15 benign prostatic hyperplasia (BPH).
