Differences in Dynamic Susceptibility Contrast MR Perfusion Maps Generated by Different Methods Implemented in Commercial Software

PurposeThere are several potential sources of difference that can influence the reproducibility of magnetic resonance (MR) perfusion values. We aimed to investigate the reproducibility and variability of dynamic susceptibility contrast (DSC) MR imaging (MRI) parameters obtained from identical source data by using 2 commercially available software applications with different postprocessing algorithms. Methods and MaterialsWe retrospectively evaluated DSC-MRI data sets of 24 consecutive patients with glioblastoma multiforme. Perfusion data were postprocessed with 2 commercial software packages, NordicICE (NordicNeuroLab, Bergen, Norway) and GE Brainstat (GE Healthcare, Milwaukee, Wis), each of which offers the possibility of different algorithms. We focused the comparison on their main analysis issues, that is, the gamma-variate fitting function (GVF) and the arterial input function (AIF). Two regions of interest were placed on maps of perfusion parameters (cerebral blood volume [CBV], cerebral blood flow [CBF], mean transit time [MTT]): one around tumor hot spot and one in the contralateral normal brain. A one-way repeated-measures analysis of variance was conducted to determine whether there was a significant difference in the calculated MTT, CBV, and CBF values. CONCLUSIONS: We found that local pathologic grade can be predicted with a high accuracy using clinical imaging data. Advanced imaging data improved this accuracy, adding value to conventional imaging. Confirmatory imaging trials are justified.

In this study, we intravenously administrated the young mitochondria into aged mice to evaluate whether energy production increase in aged tissues or age-related behaviors improved after the mitochondrial transplantation. Conclusions: Percentage of signal recovery shows low recovery values in metastases, intermediate recovery values in glioma, and overshoot in lymphoma. PSR values show lower overlap than rCBV between lymphoma and metastases; and between high grade glioma and metastases. PSR difference is also higher than rCBV between low- and high-grade gliomas. Hence, PSR can potentially help as an additional perfusion parameter in the preoperative differentiation of these tumors.

CONCLUSIONS: The sensitivity and specificity of multiphase arterial spin-labeling were similar to those of DSC perfusion irrespective of bleeding and calcification in primary brain tumors. Thus, we suggest that noncontrast multiphase arterial spin-labeling can be used instead of DSC perfusion MR imaging in the diagnosis and follow-up of intracranial tumors.

ConclusionHypoxia PET imaging has demonstrated potential benefits for grading, diagnostic and prognostic purposes, despite the lack of tangible results able to introduce such method in a wide clinical setting.
