New multidetector computed tomography quantitative technique in evaluation of shoulder instability

ConclusionMDCT is an optimal imaging modality in preoperative evaluation of glenoid and humeral head bone loss. It is the most accurate radiologic method in evaluating the new concepts of on-track and off- track Hill-Sachs lesions.

Syndesmotic injuries are common, but only a subset of these injuries are unstable. A noninvasive tool for identifying instability would aid in the selection of patients for surgery. Weightbearing computed tomography (CT) data have been reported for healthy patients, but there are limited data on unstable syndesmoses. We evaluated the syndesmotic area of arthroscopically proven unstable ankles after acute injury. This is a prospective comparative study of consecutive patients recruited to a weightbearing CT database.

ConclusionsPartial lateral meniscus anterior root injury during anatomical single-bundle ACL reconstruction was suspected in 18% of cases. Patient height and the distance between bony landmarks were significantly shorter in the injury group than in the intact group. Surgeons should understand that even a slight deviation of the tibial tunnel position can lead to partial lateral meniscus anterior root injury in patients with small skeletons.Level of evidenceIV.

ConclusionsThe rate of union at 3 months after an arthroscopic Latarjet with double-button fixation was 64%, reaching 93% at 1  year. This procedure should be carefully indicated in case of tobacco use or instability without glenoid bone defect, especially when the shoulder is exposed to high-energy trauma in the early phase after surgery.
