Abstract We retrospectively identified 89 consecutive patients from January 2004 to January 2012 to investigate efficacy of direct intrahepatic portocaval shunt (DIPS) combined with inferior vena cava (IVC) stenting for sinusoidal obstruction syndrome (SOS) associated with hepatotoxicity of pyrrolizidine alkaloids. Indications for treatment were variceal hemorrhage and/or refractory ascites. Patients were treated with DIPS plus IVC stenting (group A, n=68) or DIPS alone (group B, n=21). A technical success rate of 100% was obtained in all 89 patients, and there were no early procedure-related adverse events or 30-...

Conclusions: Compared with clopidogrel, ticagrelor appears to rapidly reduce the prevalence of inflammatory reactions and stabilize the functions of vascular endothelium to improve the stability of atherosclerotic plaque and decrease the occurrence rate of thrombosis as well as ischemic outcome events without any obvious increase in the risk of bleeding in patients with acute STEMI receiving urgent PCI. This renders it a potential drug for clinical practice. At the same time, measurement of ESM-1, a new biological marker for vascular endothelial function disorder, could possibly become a simple, effective, and practical ne...

CONCLUSION: MOC is a safe and feasible operation for symptomatic cholelithiasis when cholecystectomy is indicated. The low operative morbidity and mortality in the context of a high risk patient profile and complicated gallstone disease makes this procedure an alternative to LC where LC is inaccessible. AbstractThe aim of this paper was to demonstrate the treatment outcomes following immediate functional loading concept of short implants inserted for single tooth replacement in the posterior maxilla. The study was performed on 63 patients who received short (6  mm) implants for single tooth replacement in the posterior maxilla. Forty-eight patients underwent immediate functional concept, whereas 15 of the implants were loaded 3 months after insertion. The patients were evaluated for up to 5 years after prosthesis completion. The endpoints included the evaluation of implant survival rate, crown length, bone ...

AbstractBackgroundBleeding and obstruction are common localized symptoms in patients with unresectable gastric cancer. Although there are several reports about surgical and endoscopic therapies for gastric cancer, there are few regarding palliative radiation therapy.MethodsWe retrospectively analyzed data for 23 gastric cancer patients who underwent palliative radiation between April 2006 and March 2014, with either localized bleeding (n = 18) or upper gastric obstruction (n = 10).ResultsThe median (range) total dose and fraction (Fr) of radiotherapy (RT) were 42 (18 –60) Gy and 20 (9...

Conclusion: The procedure demonstrated a high (approximately 90%) rate of providing a sufficient sample for the diagnosis and a low complication rate, the most common complication being self-limiting bleeding.

…actually, it was probably the 27th seal. We saw quite a few on a boat trip from Hugh Town on St Mary’s to the eastern isles of Scilly aboard Sea King skippered by Fraser Hicks. There were lots of adults and pups of the large species known as the grey, or Atlantic, seal (Alichoerus grypus, meaning “hooked-nosed sea pig”). Of course, seals are more closely related to otters and bears than pigs although their resemblance to dogs, and cats even, is often commented on. It’s evolutionary convergence, I believe, although otters and bears, cats, dogs and seals all share a common ancestor if you foll...

Keith Gray, Sanjib D Adhikary, Piotr JanickiJournal of Anaesthesiology Clinical Pharmacology 2018 34(2):155-160 The field of pharmacogenomics seeks to understand how an individual's unique gene sequence can affect their response to certain drugs. It is particularly relevant in anesthesia when the interindividual response to pain medication is essential. Codeine and tramadol are prodrugs metabolized by CYP2D6, polymorphisms of which can cause dangerous or even fatal levels of their metabolites, or decrease the level of metabolites to decrease their analgesic effect. Many other opioids are metabolized by CYP2D6 or CYP3A...

A 31-year-old Chinese woman (gravida 3, para 2) presented to our hospital for requesting termination of an early pregnancy at 8 weeks. The length of her menstrual cycle was 30 days, with bleeding lasting roughly 6 to 7 days. She has been found to have situs inversus totalis (SIT) since childhood when a doctor failed to hear heart sounds in her left chest during health examination. At presentation chest X-ray revealed dextrocardia (Fig. 1). Echocardiography showed no abnormalities in cardiac morphology, structure and valve activity.
