Arthritis is a common cause of chronic musculoskeletal pain and disability; especially across ageing populations. Arthritis has traditionally been classified as either inflammatory (e.g. rheumatoid arthritis; RA) or non-inflammatory (e.g. osteoarthritis; OA, the commonest reason for joint replacement surgery). RA is a systemic, autoimmune condition with a predilection for synovial joints of the hands and feet. Synovitis (inflammation of the joint lining) contributes to pain, and also leads to joint damage.

A previously healthy, well-appearing 42-year-old female living in a modern, high-rise apartment in downtown Los Angeles calls 9-1-1 at 5:30 am complaining of worsening of a burning, epigastric pain she had been experiencing for the last three days. She reports associated nausea and non-bloody, non-bilious vomiting, and that she couldn’t manage to get comfortable in bed until she finally decided to call for help at daybreak. During her 9-1-1 call, she reports “pain, like heartburn, that just woke me up again and I had to throw up, … and then I was sweating so much.” Using the Los Angeles Tiered Disp...

This article will discuss a few factors that influence migraine (mostly episodic) and its treatment, such as sleep, obstructive sleep apnea (OSA), obesity, and affective disorders, as well as autoimmune diseases. Practitioners must be aware of these coexisting conditions (comorbidities) as they affect treatment. It is noted in literature that both the quantity (too much or too few hours) and the quality (OSA related) of sleep may worsen migraine frequency. An associated risk factor for OSA, obesity also increases migraine frequency in episodic migraine cases. A bidirectional relationship with migraine along with depression...

The Wheat Belly lifestyle begins with elimination of the worst and most dominant of all grains in the diet, semi-dwarf wheat products, followed by elimination of its closely-related brethren in other grains. This alone is a powerful start in reversing the 200-some diseases of autoimmunity. We now know that the gliadin protein of wheat and related proteins in other grains trigger increased intestinal permeability that initiates the process, as highly inflammatory compounds, such as lipopolysaccharide from bacterial cell walls, are permitted entry into the body. We also know that gliadin itself gains entry into the bloo...

Our objective is to clarify relationship between reversible cerebral vasoconstriction syndrome and administrating etanercept during puerperium. Several lines of evidence have suggested tumor necrosis factor (TNF) as a mediator of vascular dysfunction associated with estrogen deficiency. A 32-year-old woman resumed etanercept (25 mg/week), a TNF inhibitor, which had been discontinued during pregnancy, because of the deterioration of rheumatoid arthritis. She was admitted to our hospital with upper right quadrant blindness and mild right hemiparesis accompanied by pulsating left occipital pain, which had appeared 4 hours aft...
