Face transplantation -- An established option to improve quality of life in patients with severe facial trauma

(Wolters Kluwer Health) Thirteen years after the first successful face transplant, US trauma surgeons should be aware of the current role of facial transplantation for patients with severe facial disfigurement -- including evidence that the final appearance and functioning are superior to that provided by conventional reconstructive surgery. That's the message of a special update on 'Face Transplantation Today' in the June issue of The Journal of Craniofacial Surgery, edited by Mutaz B. Habal, M.D., and published in the Lippincott portfolio by Wolters Kluwer.

Conclusions This case represents a new underlying disease (trauma + GPA) leading to face transplantation and a unique clinical scenario where allografting was indicated for potentially life-threatening and sight-preserving reasons and not for mere functional and aesthetic concerns. Despite complexity, 3-year clinical outcome is encouraging, and the patient is no longer at risk for dural exposure, meningitis, and related morbidity.

Discussion: A 46-year-old female patient presented with longstanding complete facial paralysis after resection of a right acoustic neurinoma. As initial surgery, free gracilis transfer was performed on the cheek, but the patient experienced commissure movement during meals postsurgery. Secondary corrective surgery was performed to detach the motor nerve of the gracilis from the masseter nerve and suture it to the facial nerve of the healthy side via cross-face nerve graft. The symptom improved but partially recurred. Improvement in synkinetic movement can be obtained by performing cross-face nerve grafting and subsequent nerve switch.

This study aims to determine the relationship between the time of autogenous cartilage in vitro and the degree of absorption in animal experiments. Methods: New Zealand white rabbits were randomly divided into 3 groups according to the time of cartilage in vitro: 1-hour group, 2-hour group, and 3-hour group. In this study, the authors explored the roles of UC-MSCs to treat the traumatic brain injury. Umbilical cord-derived mesenchymal stem cells were isolated from healthy neonatal rat umbilical cord immediately after delivery. Conclusions: The combination of DO, orthognathic surgeries, autologous fat particle transplantation, and masseter muscle type A botulinum toxin injection technique could be a comprehensive treatment plan for adult patients of HFM. In this study, a clinical case using patient-specific 3D-printed bioresorbable scaffold with bone marrow stromal cells collected from iliac crest in the operating room has been introduced.
