Management of Unstable Middle Turbinate in Endoscopic Sinus Surgery

AbstractTo propose a simple technique for preservation, medialization and stabilization of the middle turbinate through a planned medial synechiae formation using suction electrocautery between middle turbinate and nasal septum. Study design is Case series. 150 cases of chronic rhino sinusitis who underwent ESS were selected and stabilization of the middle turbinate through a planned medial synechiae formation using suction electrocautery was done as last step and results studied. Adequate medialization was achieved in 141 cases with 94% success rate. This technique of medialization of middle turbinate through a planned medial synechiae formation using cautery between middle turbinate and nasal septum is simple, quick to perform, do not require any special or costly instrument, do not require any special training and thus can be easily done by beginners also and most importantly with good success rate and surgery outcome.

ConclusionODS caused nearly 50% of all USD cases with maxillary sinus opacification on CT, and was the most common individual etiology. ODS was frequently associated with anterior ethmoid and frontal sinus disease on CT, and middle meatal purulence on nasal endoscopy.

We report an unusual case of a 53-year-old male with fistula, ectropion and lagophthalmos due to lateral frontal rhinosinusitis. Two years ago, he presented soft erythematous swelling at internal epicanthus. A year and a half before, he presented upper eyelid fistula secretion drainage, cicatricial ectropion and lagophthalmos. In this study, we determine whether a specific HLA allele was associated with ASD in a large cohort of patients with ASD. Identifying such an association could help in the identification of immune system components which may have a causative role in specific cohorts of patients with ASD who share similar specific clinical features. ConclusionAlthough REAH is a benign disease, it is critical to reach the correct diagnosis, in order to avoid aggressive treatment. Unfortunately, the preoperative investigations were not consistent with REAH, thus it was misdiagnosed and treated aggressively.
