Insurance can be quite high, and that paper was published in 2002, so I can only imagine how much it has increased now.
They probably do earn more than doctors in the UK, but generally cost of living in the US is less than the UK. You're probably getting a lot more for your money there and can have some left over, so it can also feel like a lot more.
