Potential Morbidity Reduction With Proton Radiation Therapy for Breast Cancer

Proton radiotherapy confers significant dosimetric advantages in the treatment of malignancies that arise adjacent to critical radiosensitive structures. To date, these advantages have been most prominent in the treatment of pediatric and central nervous system malignancies, although emerging data support the use of protons among other anatomical sites in which radiotherapy plays an important role.With advances in the overall treatment paradigm for breast cancer, most patients with localized disease now exhibit long-term disease control and, consequently, may manifest the late toxicities of aggressive treatment.

ConclusionsThe review of the benchmark and rapid review QA submissions indicates that acceptable variations or unacceptable deviations for the ipsilateral lung and heart dose constraints were the most commonly observed cause of benchmark QA failure, and unacceptable deviations in target contouring, rather than normal structure contouring, were the most common cause of rapid review QA failure. These findings suggest that a rigorous QA process is necessary for high quality and homogeneity in radiation therapy in multi-institutional trials of breast cancer to ensure that the benefits of radiation therapy far outweigh the risks.

Over the past decade, several strides have been made to improve the management of breast cancer in developing countries; however, there are still obstacles present. In the area of radiation therapy, these hurdles include limited access to radiotherapy treatment and scarcity of oncology specialists. In an effort to reduce inequities in cancer care while improving patient outcomes, our research is focused on developing automated postmastectomy radiation therapy (PMRT) plans for breast cancer patients in these underserved communities that can be further improved upon through treatment planning system (TPS) specific optimization guidelines.
