Marrouche et  al (N Engl J Med 2018;378:417, PMID 29385358) randomized patients with atrial fibrillation (AF) with prior unsuccessful therapy with an antiarrhythmic drug to catheter ablation (179 patients) or medical therapy (rate or rhythm control) (184 patients). All the patients had New York Heart Associatio n class II, III, or IV heart failure; left ventricular ejection fraction ≤35%; and an implanted defibrillator. The primary end point was a composite of death or hospitalization for heart failure.
