Response to commentary re ‘Primary female epispadias: Perineal approach or Kelly repair?’

Once admitting that primary female epispadias is a heterogeneous malformation, the ultimate challenge remains to identify which tailored surgical approach can provide voided continence. The author of the commentary supports a strategy based on urethral lengthening in the first months of life to provide ‘some outlet resistance’, which unfortunately has proven insufficient in a large proportion of patients who will eventually require radical bladder neck surgery.
