Multiple Subcortical Infarcts Manifested as Isolated Bilateral Palsy of the Hypoglossal Nerve

A 50-year-old man was admitted with sudden onset of inability to speak, and severe difficulty in swallowing started 24 hours ago. He had loss of speech, but his writing and comprehension abilities were well preserved. He described an episode of mild dysarthria 20 days earlier with complete resolution after 8 days. His personal and family history were unremarkable.
