Treatment of Acetaminophen Overdose in an Adolescent Female with Delayed Presentation: A Case Report

We describe a 14-year-old female with no significant past medical history who presented 12 hours after ingesting 15 grams (291.8 mg/kg) of acetaminophen. Despite her treatment with NAC, the patient developed severe hepatotoxicity and risk of hepatic failure. Discussion NAC is less efficacious in preventing severe hepatotoxicity when given after 12–24 hours after acetaminophen ingestion. However, due to the favorable side effect profile of NAC, it is our opinion that it should be administered to all patients with known or suspected acetaminophen ingestion, despite having a delayed presentation, unknown serum concentration, or unknown time of ingestion.
