Pseudotumor sign showing retograde intussusception status postgastric bypass

A 35-year-old female, 6 weeks pregnant, with history of laparoscopic Roux-En-Y gastric bypass presented with sudden onset of epigastric and periumbilical pain associated with nausea and vomiting. Abdominal ultrasound confirmed intrauterine pregnancy. Laboratory investigations were all within normal limits. Magnetic resonance imaging revealed intussusception of small bowel with close-loop obstruction and a pseudokidney sign. Emergent laparoscopy was performed, which confirmed the diagnosis. Significantly dilated loop of jejunum approximately 20 cm in length was noticed close to the jejuno-jejunostomy with retrograde telescoping of the jejunum.
