Uremic cranial neuropathy

A 22-year-old man with rapidly progressive chronic kidney disease secondary to Alport syndrome, lost to medical follow-up for more than 1 year, presented with a 1-day history of weakness, ataxia, dysarthria, dysphagia, headache, and blurry double vision. Physical examination was notable for left-sided ophthalmoplegia (partial cranial nerve III palsy with ptosis and upgaze paresis) and facial diplegia. He was also noted to have hyperreflexia and clonus. Laboratory investigations revealed blood urea nitrogen of 75 mmol/l and creatinine of 2340 umol/l.
