We describe an adult case of purpura fulminans developing in association with AT 3 deficiency without infection following a surgical procedure, and acute kidney injury (AKI) developing secondary to rhabdomyolysis and disseminated intravascular coagulation (DIC). To the best of our knowledge there have been no previous cases of purpura fulminans and AKI developing in association with acquired AT 3 deficiency without infection after surgery. Conclusion ER in the civilian population occurs at a much lower incidence than the military population. The most common causes were endurance exercise, manual labor, and weight lifting. The majority of cases were treated conservatively with intravenous fluid resuscitation during a brief hospital stay, and all were discharged without sequela. Only one case of recurrence occurred in this cohort, indicating the recurrence rate was low.

Poisoning caused by a large range of drugs and chemicals can induce metabolic complications via a large number of mechanisms. Some metabolic complications are life-threatening but all require careful assessment, appropriate monitoring and consideration of treatment. Prompt diagnosis and management reduce morbidity and mortality. Treatment may be targeted to the specific poisoning, and may be time-critical, or can follow similar approaches to those of general and acute medicine. This review introduces metabolic complications associated with sodium, potassium, metabolic acidosis, rhabdomyolysis and methaemoglobinaemia.

LPIN1, located on chromosome 2p25.1, encodes lipin-1, an enzyme abundantly expressed in adipose tissue and muscle . Lipin-1 acts as a phosphatidic acid phosphatase (PAP) catalysing the conversion of phosphatidic acid to diacylglycerol in the triacylglycerol synthesis pathway . It is therefore an important enzyme in the biosynthesis of triglycerides and phospholipids. Lipin-1 also serves as a transcriptional coactivator, regulating expression of genes involved in fatty acid ß-oxidation and mitochondrial respiratory chain enzymes .
CONCLUSIONS: The LDL cholesterol, total cholesterol, and triglyceride lowering effect of cerivastatin was linearly dependent on dose. Cerivastatin log dose-response data were linear over the commonly prescribed dose range. We present a case of rhabdomyolysis related to treatment with sorafenib in a patient with multifocal hepatocellular carcinoma. Rhabdomyolysis is a severe situation and potentially fatal. There are no laboratory data that can predict this condition. Early diagnosis is essential for prognosis.
