We would like to address concerns raised to us by Dr. Jean-Fran çois Brunet, from Institut de Biologie de L'Ecole Normale Supérieure (Ibens), France, whose article has been cited in our paper (ref number 8, [1]). Based upon developmental programs and a panoply of transcription factors expressed during development, Dr Jean-François Brunet and colleagues conclu de that the sacral autonomic outflow is sympathetic rather than parasympathetic. The sacral autonomic outflow is, however, dissimilar from the classical sympathetic system with respect to their pharmacology.

Male sexual dysfunction after aortoiliac surgery is a distressing complication. Erectile dysfunction and ejaculatory function postoperatively are mainly related to the methods of exposure and dissection of the aorta and iliac arteries because it is the preferred passage area of the upper hypogastric plexus, the main crossroads of the pathways of sympathetic nerves with pelvic intent. The purpose was to determine the incidence of erectile dysfunction in patients with aortoiliac occlusive disease and to compare the change in erectile function after open surgery and endovascular repair.

ConclusionCurrent literature includes studies of highly heterogeneous populations, consuming differing volumes of alcohol over variable periods of time and utilising a number of different autonomic test batteries and criteria to diagnose autonomic dysfunction. Therefore, further research using homogeneous methods for measuring autonomic dysfunction in the field is needed. Despite this limitation, our review demonstrated that autonomic dysfunction is very common among alcohol abusers.

Conclusions Dysautonomic symptoms frequently occuring in α-synucleinopathies comprise cardiovascular, gastrointestinal, urogenital and thermoregulatory disturbances. These symptoms reduce quality of life and worsen prognosis.
