'This is not a cosmetic surgery,' says Manitoba woman fighting for coverage of out-of-country treatment

A Winnipeg woman says her quality of life has drastically improved since going to Germany for water-assisted liposuction to remove painful fat nodules caused by a chronic disease. But the province refuses to pay for the treatment, saying there isn't adequate proof that it's effective.

The aim of this study was to compare the quality of postburn facial scars before and after injection of unfiltered nanofat. The study was performed in the Plastic Surgery Department of Mayo Hospital, Lahore, Pakistan, from January 2015 to December 2016. Forty-eight patients with postburn facial scars were included; age range was 4 to 32 years with Fitzpatrick skin types between 3 and 4. Patients with hypertrophic scars, contractures, or keloids were excluded. Scars were assessed by a senior plastic surgeon and the patient on the POSAS (Patient Observer Scar Assessment Scale). Fat was harvested from the abdomen and/or thigh...

Conclusions Assuming that the extrinsic factors influencing fat function and distribution remain stable, capabilities of the redistributed fat to handle free fatty acid and store lipid substance are reduced, leading to substantial tissue atrophy and volume decline after grafting.

AbstractPurposeThe aim of this prospective, randomized, double-blind study was to compare the tumescent anesthesia method and erector spinae block with respect to postoperative analgesia consumption, pain scores and patient satisfaction, in patients receiving breast reduction surgery under general anesthesia.MethodsThe study included 44 females, aged 20 –65 years, who were to undergo breast reduction surgery, without adjunctive liposuction on the breast. Using the closed envelope method, the patients were randomly separated into two groups to receive tumescent anesthesia or erector spinae block (ESB). Patients i...

ConclusionsLipoabdominoplasty is a valuable tool to perfect body shape. Conclusions: Recent concerns regarding buttock injections have been raised due to death from fat embolism. Despite numerous theories on the mechanism of entry, the fact that venous injury occurs with the introduction of boluses of fat in enough quantities to cause mechanical disruption of the cardiopulmonary circuit is not in dispute. The authors believe this cannot occur in the awake patient without the surgeon being acutely aware of a misguided cannula. These data suggest that buttock fat grafting under local anesthesia is a safe and effective procedure with an excellent safety profile and durable results. CLINICAL QU...

Conclusions: When encountering a patient with a history of medical tourism and treatment-refractory infection, rapidly growing mycobacteria must be considered. To increase the likelihood of yielding a diagnostic organism, multiple acid-fast bacilli cultures from fluid and débridement content should be performed. There has been reported success in treating rapidly growing mycobacterial infections with a combination of antibiotics including clarithromycin, amikacin, and moxifloxacin.

Adipose tissue represents an abundant source of regenerative cells, easily accessible through liposuction. Lipoaspirates are used in the clinic either after enzymatic digestion into stromal vascular fraction or, as of more recently, as mechanically microfragmented fat. We now show that microfragmentation increases pericyte content, compared to plain lipoaspirate, and augments growth factor and cytokine secretion compared to stromal vascular fraction. AbstractAutologous adipose tissue is used for tissue repletion and/or regeneration as an intact lipoaspirate or as enzymatically derived stromal vascular fraction (SVF), which...

The cervicofacial lift is a frequently performed procedure in plastic surgery. It is the reference technique for facial rejuvenation and restoration of the oval form of the face and it is essential to treat excess skin. One of the most frequent complications of this procedure is the formation of haematomas. The aim of this article is to analyze the incidence of bleeding in the standard cervicofacial lift carried out under pure local anaesthesia. The entire operation, including liposuction and tightening of the SMAS, can be done under local anaesthesia.

Body contouring procedure is a catchall that includes mastopexy, abdominoplasty, liposuction, and lower body lift. It is well known that optimal postoperative pain control decreases hospitalization length of stay and promotes early ambulation, which in turn decreases the incidence of complications. There have been many innovations in pain control beyond opiate medications, including prolonged administration of local anesthesia, regional blocks, and the use of a multimodal regimen which utilizes nonopiates. All approaches aim to decrease oral or intravenous opiate use and, therefore, encourage faster return to normal bowel ...
