Better advance care planning in long-term care through starting the conversation

Due to cognitive impairment, multimorbidity, and fewer family connections, nursing home residents are at especially high risk for receiving medical care that is medically inappropriate or inconsistent with their wishes. Therefore, Advance Care Planning (ACP) in this setting is crucial to the provision of patient-centered care. The Transportable Physician Orders for Patient Preferences (TPOPP) form has been developed and implemented in Kansas and Missouri to allow patients to discuss ACP and implement orders to ensure that their wishes are respected across the entirety of the healthcare system.
