Measuring Team Effectiveness in the Veterans Health Administration's Home-Based Primary Care Program

The Department of Veterans Affairs (VA) operates Home-Based Primary Care (HBPC), a program designed to provide comprehensive and longitudinal care to community-based frail Veterans with complex, chronic care needs. The HBPC has been in existence for over 40 years and encompasses almost 400 sites enrolling 50,000+ Veterans. To date, much of the HBPC-related research has focused on the program ’s impact on cost of care and clinical quality. Little is known about variations in HBPC interdisciplinary teams’ functioning.
