Diffuse liver metastasis is a rare pattern of liver metastasis that is associated with hepatic failure and poor prognosis. We experienced 2 cases of acute liver failure due to diffuse metastasis that could not be detected using computed tomography. Incase 1, it was difficult to differentiate diffuse metastasis from alcoholic hepatitis. Incase 2, it was difficult to diagnose diffuse liver metastasis because the patient had no history of malignancy. When liver enzyme levels are elevated, it is necessary to consider liver metastasis as a potential cause, regardless of computed tomography findings.Case Rep Oncol 2018;11:699 –704

We present the case of a 67-year-old woman affected by hypertension and hypercholesterolemia. She had a history of alcoholism, anxiety disorder with panic attacks, and paroxysmal atrial fibrillation. Despite a previous transient ischemic attack and her thromboembolic risk (CHA2DS2-VASc score: 5), she had not been taking anticoagulants at home without a reasonable motivation. Moreover, around eighteen years prior, she had breast cancer and underwent surgical intervention, radiotherapy and several years of hormone-therapy.

ConclusionPatients who received radiotherapy and smoked before or at time of BC diagnosis have an increased risk for specific SPCs; drinking alcohol did not alter the effect of radiotherapy. Smoking significantly increased mortality risk reducing the protective effect of radiotherapy treatment.

CONCLUSIONS: Our study suggests that PTX up to 100 mg/m2 can be used safely for patients with the ALDH2 GA genotype. To confirm the necessity of a genotyping test for ALDH2, further studies evaluating alcohol sensitivity in response to PTX among patients with the ALDH2 AA genotype are required.
