I Ching Balance Acupuncture Treatment of Migraines, Insomnia, and Depression in a Post-Ovariectomy Patient Receiving Hormone Replacement Therapy.

Conclusions: This patient reported a considerable improvement in all the outcome measures. She completed the treatment after experiencing great improvement in her QoL. Additional, larger-scale studies are warranted to investigate the effects of ICBA. PMID: 29410721 [PubMed]

Abstract The molecular pathogenesis and classification of colorectal carcinoma are based on the traditional adenomaecarcinoma sequence, serrated polyp pathway, and microsatellite instability (MSI). The genetic basis for hereditary nonpolyposis colorectal cancer is the detection of mutations in the MLH1, MSH2, MSH6, PMS2, and EPCAM genes. Genetic testing for Lynch syndrome includes MSI testing, methylator phenotype testing, BRAF mutation testing, and molecular testing for germline mutations in MMR genes. Molecular makers with predictive and prognostic implications include quantitative multigene reverse transcriptas...

CONCLUSIONS: Palliative sedation is an alternative to alleviate end-of-life patient suffering due to refractory symptoms, particularly dyspnea and delirium, after all other treatment options have been exhausted. Decision making involves prior explanations, discussions and agreement of the team, patient, and/or family members. It can be performed in general hospital units, hostels and even at home. Midazolam is the most indicated drug, and neuroleptics may also be required in the presence of delirium. These patients' monitoring is limited to comfort observation, relief of symptoms, and presence of adverse effects. There is ...

CONCLUSION: Since cardiac tamponade is generally not suspected as a cause of hemodynamic instability after a lung lobectomy, as it was in this case, a misdiagnosis of the patient's condition may have led to improper management resulting in death. As anesthesiologists are often involved in the initial resuscitation of morbid patients in Post-Anesthesia Care Units, their acquaintance with various postoperative complications and competence in echocardiography for assessing cardiac problems may contribute to patient survival. A 69-year-old man was referred to our department because of renal masses of the left kidney. The patient was diagnosed with a bladder carcinoma 10 months before in the context of macroscopic hematuria. The cancer had a partial squamous and papillary differentiation and was staged as G3pT1B with the infiltration of the superficial part of the bottom of the chorion. The patient underwent a transurethral resection of the bladder and had a course of intravesical injections of Bacillus Calmette-Guerin (BCG).

A 3-year-old female presented with 4 months of recurrent fevers, abdominal pain, hematuria, and proteinuria with negative urine cultures. On examination, she was hypertensive with bruises over her left cheek. Labs were remarkable for serum creatinine 0.78 mg/dl (estimated glomerular filtration rate of 48.3 ml/min per 1.73 m2 using the revised Schwartz formula); hemoglobin 8.5 gm/dl; hematocrit 27.9%; platelet 92 × 103/ul; urinalysis with 11–20 red blood cells/high-power field; spot urine protein/creatinine ratio 1.97 mg/mg; low haptoglobin; elevated lactate dehydrogenase; and schistocytes on peripheral smear.

High incidence of BRAF V600 mutation in Indian patients with head and neck cancer. Front Biosci (Elite Ed). 2018 Jun 01;10:520-527 Authors: Gauthaman A, Moorthy A Abstract In cancer cells, BRAF is frequently mutated at codon 600 (V600) leading to the replacement of valine amino acid with other amino acids. The current study was performed to assess the prevalence of BRAFV600 mutation in Indian patients with head and neck squamous cell carcinoma (HNSCC). Among the patients, 27% were homozygous, and 71% were heterozygous for the mu...

Authors: Li S, Gao M, Li Z, Song L, Gao X, Han J, Wang F, Chen Y, Li W, Yang J Abstract Chemoresistance is a critical obstacle to the treatment of hepatocellular carcinoma (HCC). The mechanisms underlying resistance to doxorubicin, cisplatin, and 5-fluorouracil involve p53 and P-glycoprotein (P-gp). p53 plays a role in cell growth; therefore, resistance mechanisms involve chemotherapy-induced apoptosis and p53 mutation and inactivation. P-gp is an energy-dependent drug efflux pump regulated by p53. Its role in drug resistance has provided new insights into the mechanisms underlying the involvement of p53 and P-gp i...
