• Leukemic transformation of donor hematopoietic stem cells provides a useful model to study the mechanisms involved in leukemogenesis.• A systematic analysis of 137 cases reported in the literature highlights several factors that may contribute to the development of donor cell derived hematolog ical neoplasms after allo-SCT.
