Association Between Inflammatory Back Pain Characteristics and Magnetic Resonance Imaging Findings in the Spine and Sacroiliac Joints

ConclusionThe identified associations between inflammatory MRI findings and pain characteristics indicate that axial inflammation to some degree induces a specific pain pattern. Thus, the results add to knowledge of axial inflammatory processes. However, all identified associations were weak, which compromises the use of IBP as a marker of axial inflammation.

ConclusionIn this population, 53% had at least 1 clinical feature included in the ASAS criteria for SpA, and 21% had sacroiliitis according to the ASAS definition; furthermore, the associations between the clinical and imaging domains were inconsistent. The results indicate a need for further investigation of the importance of these findings in SpA, including investigation of the minimum requirements for defining sacroiliitis on MRI.

We present the case of a 63-year-old male with right calcaneal osteomyelitis who developed acute onset lower back pain with concomitant fever and chills. A 13-year-old female admitted to our pediatric neurology department with the complaints of difficulty in walking, left leg pain and lower back pain ongoing since one month. Having the symptoms of lumbosacral radiculopathy, a magnetic resonance imaging (MRI) was ordered to exclude any herniated disc, spinal stenosis or arthritic/degenerative changes of the facet joints. Lumbosacral MRI demonstrated a 4 cm × 3,5 cm × 3,5 cm well defined, expansile lesion of the left side of the sacral bone at the level of S1-S2 vertebrae extending to the left sacroiliac joint (Figure 1, Figure 2).

Conclusion: This study has demonstrated a negative dose–response relationship between increasing severity of disc degeneration with a reduction in intervertebral disc height. Although the assessment of disc degeneration incorporates a number of qualitative measures, these data substantiate the utility of intervertebral disc height as a quantitative and continuous outcome measure in epidemiological studies, and potentially clinical practice.
