Fast growing melanoma following treatment with vismodegib for locally advanced basal cell carcinomas: report of two cases

A 82-year-old woman with histopathologically proven, locally advanced, multifocal, infiltrative basal cell carcinomas (BCCs) of the centro-facial area and the d écolleté was referred to the skin cancer unit, Medical University of Graz (Austria). She was not considered a candidate for surgery or radiotherapy, so she started treatment with the hedgehog (HH) inhibitor vismodegib (150 mg/day orally). Owing to increasing dysgeusia and muscle cramps, the dosag e of vismodegib was continuously reduced after the first 3 months to 150 mg every second day (month 4), to every third day (month 5) until twice/week for months 6–8, with a decrease of the severity of symptoms and complete clinical and dermoscopic response.
