Cerebral Toxoplasmosis Diagnosed by Nested-Polymerase Chain Reaction in a Patient with Rheumatoid Arthritis. Intern Med. 2018 Jan 11;: Authors: Matsuura J, Fujii A, Mizuta I, Norose K, Mizuno T Abstract A 65-year-old woman with rheumatoid arthritis (RA) visited our hospital because of right facial sensory hypoesthesia. Cerebral toxoplasmosis was suspected on brain magnetic resonance imaging. We discontinued methotrexate for RA and started a sulfamethoxazole/trimethoprim (ST) mixture. Although ST treatment was interrupted because of adverse reactions, her prognosis was favorable. The Toxoplasma 18S rDNA gene was detected by nested-polymerase chain reaction (PCR) from blood and cerebrospinal fluid. Detecting the Toxoplasma 18S rDNA gene by nested-PCR is useful for the diagnosis and safer than a brain biopsy. In addition, the discontinuation of immunosuppressants may be recommended in patients compromised by those immunosuppressants. PMID: 29321415 [PubMed - as supplied by publisher]

CONCLUSION: Cardiac manifestations in IJD are frequent and they are the leading cause of an increased morbi-mortality in IJD. Clinicians would be aware of that, given that early diagnosis of these complications may reduce the frequency of CV events and improve survival of patients with IJD. In conclusion, next to decreasing disease activity and joint erosions by suppressing in flammation, anti-TNF therapy in RA can also suppress potential CNS involvement linked to BBB (blood–brain barrier) dysfunction. Further studies with broader participation and longer patient follow-up are needed to reinforce this hypothesis.
