This study retrospectively investigated the efficacy of the ESP block in pain management after lumbar spinal surgery. Methods: Patients who underwent lumbar spinal surgery in 2017 were enrolled in the study. Those who underwent secondary surgery with local anesthesia other than the ESP block were excluded. The primary outcome was the Numerical Rating Scale (NRS) pain score at various time points until the morning of postoperative day 2. THE AMERICAN SOCIETY OF REGIONAL ANESTHESIA (ASRA) and Pain Medicine recently published updated guidance on the management of local anesthetic systemic toxicity (LAST).1,2 Organ systems that are primarily disrupted by toxic doses of local anesthetics include the cardiovascular system and central nervous system (CNS). Mild prodromal symptoms may progress to major complications such as seizures, cardiac arrest, or death. Treatment is multifactorial and involves the use of 20% lipid emulsion therapy as the antidote.

ConclusionsPatients with mucopolysaccharidosis are at high risk for anesthesia-related complications and require a high level of attention. However, a multidisciplinary approach, combined with expertise in the use of new techniques and new devices for airway management, makes anesthesiological management safer. Further research with prospective studies would be useful.

ConclusionManipulation under general anaesthesia is a well-recognised treatment for adhesive capsulitis. We report that targeted ultrasound-guided injection of the rotator interval and manipulation of the shoulder under local anaesthetic blockade result in good outcomes in reducing shoulder pain and symptoms of adhesive capsulitis with low recurrence and complication rates.
