Characterized mainly by irregular menses, hirsutism, skewed LH: FSH ratios and bulky polycystic ovaries, this multifactorial endocrinopathy results in unfavorable reproductive and metabolic sequelae, including anovulatory infertility, type 2 diabetes, metabolic syndrome and cardiovascular disease in later years. Increasing evidence has shown that the manifestation of polycystic ovary syndrome (PCOS) is attributable to a cumulative impact of altered genetic, epigenetic and protein profiles which bring about a systemic dysfunction. While genetic approaches help ascertain role of causal variants in its etiology, tissue-specific epigenetic patterns help in deciphering the auxiliary role of environmental, nutritional and behavioral factors. Proteomics is advantageous, linking both genotype and phenotype and contributing to biomarker discovery. Investigating molecular mechanism underlying PCOS is imperative in order to gain insight into the pathophysiology of PCOS and formulate novel diagnostic and treatment strategies. Polycystic ovarian syndrome (PCOS) has been recognized as the most common cause of infertility in women and is associated with other comorbidities including obesity, type 2 diabetes, and metabolic syndrome (Ib áñez et al., 2017; Sebastian, Wiemann, Bacha,&Alston Taylor, 2018). Hyperandrogenism with a persistent elevation of serum testosterone is a cardinal sign of PCOS and is the underlying hormonal cause of chronic anovulation, infertility, acne, and hirsutism (Rosenfield, 2015a). The prevalence of PCOS is 6% to 10% of reproductive women and is steadily increasing (Sebastian et al., 2018).

Polycystic ovary syndrome (PCOS) is a common endocrine disorder associated with poor reproductive outcomes, including infertility and pregnancy complications, and poor metabolic outcomes, including insulin resistance, type 2 diabetes, and the metabolic syndrome. Prevalence estimates for PCOS range from 7% to 15% of reproductive-age women worldwide, depending on the diagnostic criteria used. Accordingly, PCOS-associated dysglycemia and obesity are a global public health risk. Despite the public health importance of PCOS, notable gaps exist in the literature regarding its pathophysiology, natural history, and optimal management.

Conclusion This study established a significant yet independent association of PCOS with major cardiovascular risk factors. This association can effectively progress into CVD outcomes which necessitates early intervention programs and preventative strategies to reduce mortality from cardiovascular events.
