Atrial fibrillation is the most common sustained cardiac arrhythmia in the general population. In western countries with aging populations, atrial fibrillation poses a significant health concern, as it is associated with a high risk of thromboembolism, stroke, congestive heart failure, and myocardial infarction. Thrombi are generated in the left atrial appendage, and subsequent embolism into the cerebral circulation is a major cause of ischemic stroke. Therefore, patients have a lifetime risk of stroke, and those at high risk, defined as a CHA2DS2-VASc2 (congestive heart failure, hypertension, age>75 years, diabetes mel...

Authors: Meregildo ED Abstract Neurocysticercosis (NCC) is a global health problem. In more developed countries, NCC is mainly a disease affecting immigrants. In developing countries, NCC is the most common parasitic disease of the nervous system and the main cause of acquired epilepsy. NCC is also an unrecognized cause of strokes and could account for 4%-12% of strokes. Here, I report a case of a 58-year-old woman who presented to the emergency department (ED) with severe headache, vomiting, and sudden loss of consciousness. Multiple NCC and Fisher grade 4 aneurysmal subarachnoid hemorrhage (SAH) were demonstrated...

CONCLUSIONS: Inflammatory markers like NLR, LMR and CAR on admission were associated with increased risks of 30-day mortality and 90-day poor outcome in patients with critically ill stroke. Especially, high NLR is independently associated with 90-day poor outcome. PMID: 32486607 [PubMed - as supplied by publisher]

(NIH/National Institute of Neurological Disorders and Stroke) In a nationwide study, NIH funded researchers found that the presence of abnormal bundles of brittle blood vessels in the brain or spinal cord, called cavernous angiomas (CA), are linked to the composition of a person's gut bacteria. Also known as cerebral cavernous malformations, these lesions which contain slow moving or stagnant blood, can often cause hemorrhagic strokes, seizures, or headaches.

CONCLUSIONS: Patients with diabetes mellitus were vulnerable to hemorrhagic transformation, whereas those who underwent several attempts of thrombectomy were susceptible to isolated subarachnoid hemorrhage. Both hemorrhage types worsened the functional outcome. Treatment with the stent retriever was significantly associated with postprocedural isolated subarachnoid hemorrhage. AbstractBackground and purposeRandomized controlled trials have demonstrated that mechanical thrombectomy (MT) could provide more benefit than standard medical care for acute ischemic stroke (AIS) patients due to emergent large vessel occlusion. However, most primary stroke centers (PSCs) are unable to perform MT, and MT can only be performed in comprehensive stroke centers (CSCs) with on-site interventional neuroradiologic services. Therefore, there is an ongoing debate regarding whether patients with suspected AIS should be directly admitted to CSCs or secondarily transferred to CSCs from PSCs. This meta-analysis was aim...

The association between CHADS2 score and stroke has been confirmed in patients with atrial fibrillation. But the majority of cardiovascular events occurred primarily in population without atrial fibrillation. Therefore, we performed this cohort study to evaluate the association between baseline CHADS2 score and cardiovascular events in the population without atrial fibrillation. A total of 99,755 Chinese adults with complete baseline data were followed for up to 10 years. The primary outcomes were cardiovascular event (a composite endpoint of myocardial infarction, cerebral infarction and cerebral hemorrhage) and all-cause death.
