In conclusion, elderly onset DRPLA presents as cerebellar ataxia alone in the early stage of disease. Atrophy of the brainstem, superior cerebellar peduncle, and cerebellum and abnormal signals in the brainstem, cerebellum, and thalamus are key findings fo r differentiating elderly onset DRPLA from asymptomatic cerebral white matter lesions in healthy subjects.
