Clinical insights: Addressing command hallucinations

Dugre et al. (2017) highlight the clinically relevant problem of command hallucinations (CH). Clinicians may be asked to assess risk posed by CH, a symptom which may prompt substantial concern. In this context, it is important to consider that both under-reaction and over-reaction to CH can result in negative consequences, and to incorporate an evidence-based understanding of how CH may impact risk of engaging in harmful behavior. Methodological flaws weaken existing research examining the association between CH and risk of harm (e.g., Braham et al., 2004), including often small sample sizes.
