Postoperative Spinal Subdural Lesions Following Lumbar Spine Surgery: Prevalence and Risk Factors. Purpose: To clarify the prevalence and risk factors for spinal subdural lesions (SSDLs) following lumbar spine surgery. Overview of Literature: Because SSDLs, including arachnoid cyst and subdural hematoma, that develop following spinal surgery are seldom symptomatic and require reoperation, there are few reports on these pathologies. No study has addressed the prevalence and risk factors for SSDLs following lumbar spine surgery. Methods: We conducted a retrospective analysis of the magnetic resonance (MR) images and medical records of 410 patients who underwent lumbar decompression surgery with or without instrumented fusion for degenerative disorders. SSDLs were classified into three grades: grade 0, no obvious lesion; grade 1, cystic lesion; and grade 2, lesions other than a cyst. Grading was based on the examination of preoperative and postoperative MR images. The prevalence of SSDLs per grade was calculated and risk factors were evaluated using multivariate logistic regression analysis. Laminoplasty is a common surgical method used to treat patients with cervical ossification of the posterior longitudinal ligament (OPLL). Although laminoplasty is preferred over traditional laminectomy, the factors affecting the complications and outcomes are unclear. We describe the use of high-frequency SCS to alleviate the patient's post-laminectomy pain as well as his abdominal pain. Our case describes SCS use with multi-level lead placement targeting both post-laminectomy pain and abdominal pain. We describe a strategy that can be useful to patients with concurrent pain from more than one source. Surgery for degenerative lumbar disease such as lumbar laminectomy and discectomy are among the most common procedures performed in the United States (US). The incidence of laminectomies in the US increased by 11.3% over a 10-year period from 1998 to 2008 [1]. Similarly, the incidence of lumbar fusion surgery has increased by 55% from 2005-2014 [2]. Lumbar fusion alone experienced a 3.3-fold increase in cost compared with the 2.3-fold increase in cost associated with joint replacement, resulting in a $33.9 billion national expense in 2008 [1].
