Complete tear-off of a Riata dual coil lead tip

We report the case of a young woman who incidentally presented with a complete tear-off of the tip of a Riata dual coil lead while the device had reached ERI criteria. Years ago, an additional pace/sense electrode had been added due to an exit block defibrillation functioning properly. Although defibrillation properties were still adequate at the time of generator exchange, we decided to revise the lead surgically.
