Simultaneous laparoscopic sleeve gastrectomy, hiatal hernia repair, hepatic cavernous hemangioma resection and umbilical hernia repair

We are presenting a 53 year old female with a BMI of 57.   Past medical history includes OSA on CPAP, HTN, CHF, low back pain, osteoarthritis, IBS, bipolar disorder, seizure disorder and a left hepatic hemangioma that is relevant for this case. The patient had prior abdominal surgeries including laparoscopic cholecystectomy, appendectomy and 2 C-section s. She was evaluated for her morbid obesity and comorbidities and was deemed to be a good candidate for bariatric surgery.  Her preoperative workout included an upper endoscopy that was normal without esophagitis.  As a work up for an episode of epigastric and left upper quadrant pain, a CT scan was performed and revealed a 6 cm exophytic mass in the left hepatic lobe in the gastro-hepatic space.  This prompted a MRI that confirmed that the mass was a 6.5 cm hemangioma that was overlying the gastro esophageal junction.

CONCLUSIONS: Small bowel obstruction resulting from LAGB tubing is an uncommon complication which was reported in few cases. Although bariatric surgery currently represents the best treatment option for morbid obesity and its related- diseases, peri- and post-operative complications have always to be taken into account. Conclusion: Moderate quality evidence suggests that OMTh is effective for CNCP patients. There was a significant association between visceral OMTh and reduced pain severity and disability. More robust, high-quality randomized controlled trials with larger sample sizes are required to further explore the effectiveness of the OMTh in the management of CNCP. We report a patient with glucagonoma differentiation into a malignant form after 13 years. A 63-year-old asymptomatic man was evaluated for an enhancing lesion at the distal pancreas. Physical examination was normal. Laboratory values were normal except for an elevated serum glucagon level (206 pg/mL, Ref. 50 –150). Somatostatin and other tumor markers were normal.
